1. Filled Personal Data Sheet (CSC Form 212, downloadable from the www.csc.gov.ph
2. PSA – Issued Birth Certificate (Photocopy)
3.Official Transcript of Records (Photocopy)
4.College Diploma (Photocopy)
5.Certificate of Eligibility (Photocopy)
6.Application letter addressed to ISIDRO S. LAPEÑA, PhD, CSEE, Director General, Philippine Drug Enforcement Agency, NIA Northside Road, National Government Center, Barangay Pinyahan, Quezon City.
Send it to PDEA Academy c/o Recruitment Team, Camp General Mariano N. Castañeda, Brgy. Tartario, Silang, Cavite
